To accomplish this, we work hand in hand with you to maximize your insurance reimbursement for covered procedures. Please bring your insurance information with you to the initial appointment so that we can expedite reimbursement.

For your convenience we accept cash, checks, and most major credit cards. Payment is due at the time service is rendered. As a courtesy to you, we will accept direct payment from your insurance company for the portion of the fee that they pay, if you have an insurance plan with which we participate.

We will contact your insurance carrier in an effort to estimate your coverage. Based upon information received from your insurance carrier, we will approximate the amount of your co-payment (the portion that is your responsibility). This amount is due at the time of treatment; we cannot do post-treatment billing. The exact amount of your co-payment cannot be determined until we receive payment from your insurance carrier. If there is a difference between the estimated and actual co-payment, we will either issue a refund or notify you of any additional amount due.

Please be aware that dental insurance is a contract between you, your employer and the insurance company. Questions concerning coverage, deductibles, co-payments or yearly maximums should be directed to your employer or insurance carrier.

Please note that certain insurance carriers do not provide coverage for endodontic consultations when no treatment is provided. Such consultation fees are the responsibility of the patient.
