To make your first appointment go as smoothly as possible, keep the following in mind:

1. Print and complete the new patient forms available on our website.

2. Bring a list of current medications and alert the staff to any medical conditions you have.
3. Bring insurance information and complete any necessary forms available on our site.
4. You can request your primary dentist to forward us copies of any dental x-rays taken in the past six months.
5. Please ask your dentist to email the radiographs to our office at Garden City Endodontics
